• Comprehensive written evaluation of needs with professional recommendations for care and services.

• Specialized assessment of needs for Alzheimer’s and dementia.

• Coordination and monitoring of in-home and/or community services.

• Follow-up/ongoing visits to evaluate care, needs and services

• Referrals to legal and financial counsel and community professionals as needed.

• Written status reports and updates on progress, changes or problems

• Liaison with family, community and medical services.
