Joanne Fontaine, RN, BSN, has served seniors and their families in both community and residential facilities for over 20 years. She received her Bachelor of Science in Nursing from the University of Lowell. Joanne founded Pro-Active Eldercare, a private geriatric care management company, to provide support and resources to aging adults and their caregivers with special focus on those with Alzheimer’s Disease. Recognized as an expert in her field, Joanne is often asked to speak on the subject of Alzheimer’s disease. Joanne was instrumental in the re-design and management of Alzheimer’s Units for several Massachusetts nursing homes and continues to consult privately to area facilities. She is a member of the National Association of Professional Geriatric Care Managers and the Alzheimer’s Association. She is the past Vice Chair of the Alzheimer’s Association, Massachusetts Chapter. In her pursuit to improve the quality of life for those with Alzheimer’s disease, she has developed a successful line of activity kits for people with memory loss aptly named Alzheimer’s Survival Kits®.
Michele Hughes, RPT

Michele Hughes joins Pro-Active Eldercare with more than 25 years of experience as a registered physical therapist in a variety of health care settings. She graduated from Boston University and holds a Certificate in Gerontology from the University of Massachusetts in Boston. Michele has worked for many years in the private home care setting and most recently worked with the federally funded Caregiver Support Program at Minuteman Senior Services as the Caregiver Specialist and Program Manager. Michele has extensive knowledge of home modifications and adaptive equipment as well as disease process and chronic illness. She displays tremendous skills in assessing each client and their unique situations and designing care solutions to meet those needs.
